Office Hours and Appointments
Our regular office hours are Monday through Friday 8:00 a.m. – 5:00
p.m. Office visits are by appointment only. If you are unable to keep
your appointment, we ask that you give our office 48 hour notice. This
will allow us to schedule another patient in this place. Patients
waiting to receive medical attention will appreciate the opportunity.
We do our best to meet your scheduling needs. However, patients do
not become ill on schedule. Also, durations of surgeries cannot always
be accurately predicted and emergencies do occasionally arise.
Billing and Insurance
For your convenience, we accept personal checks, cash, Mastercard
and Visa cards. Co-payments and your share of cost for in office
procedures are due at the time of service. Your contractual portion of
surgery fees is due one week prior to scheduled surgery date. Should
this cause a financial hardship, we ask that you contact our office
manager prior to your scheduled appointment.
Our billing service will bill up to two insurance plans for services rendered.
Our Privacy Practices
We understand the importance of privacy and are committed to
maintaining confidentiality of your medical information. We are
required by law to maintain the privacy of protected health information
and to provide individuals with our legal duties and privacy practices.
This notice describes how we may disclose your medical information. Our
policy is posted in our reception area for your convenience as well as
in written hand-out format for you to refer to at your convenience.
